VitalCare Medical Spa may offer weight-loss-related services that can include medical consultation, wellness guidance, body composition support, lifestyle recommendations, provider evaluation, prescription coordination, medication management, injections, compounded medications, and follow-up care when medically appropriate.
Weight loss services are not suitable for every patient. All patients must complete required intake forms, disclose accurate medical history, list all medications and supplements, identify allergies, and complete any required medical evaluation before beginning treatment.
Weight loss results are not guaranteed. Individual results vary based on many factors, including age, metabolism, diet, exercise, medical history, medications, hormone status, sleep, stress, adherence to instructions, genetics, and underlying medical conditions. VitalCare Medical Spa does not promise a specific amount of weight loss, body fat reduction, appetite change, metabolic improvement, or timeline of results.
Certain weight loss services may require evaluation, approval, prescription, or supervision by a licensed healthcare provider. VitalCare Medical Spa reserves the right to deny, delay, modify, or discontinue treatment if the provider determines that treatment is not medically appropriate or safe.
A patient-provider relationship is established only after proper evaluation, acceptance for care, and completion of required documents and consent forms.

Compounded drugs are not FDA-approved, which means the FDA does not review them for safety, effectiveness, or quality before they are marketed. VitalCare Medical Spa does not guarantee availability of any specific medication, compounded medication, brand-name medication, generic medication, dosage, pharmacy, or formulation.
Patients are responsible for providing complete and truthful information, including:
Current medications
Supplements
Allergies
Medical conditions
Pregnancy or breastfeeding status
History of pancreatitis, gallbladder disease, kidney disease, thyroid disease, diabetes, eating disorders, gastrointestinal conditions, or other relevant medical history
Prior reactions to medications or injections
Any emergency symptoms or side effects during treatment
Failure to disclose accurate health information may increase risk and may result in cancellation, denial of treatment, loss of deposit, or termination of services.
Depending on the treatment or medication used, possible risks or side effects may include nausea, vomiting, constipation, diarrhea, stomach pain, appetite changes, dehydration, dizziness, fatigue, headache, injection-site irritation, allergic reaction, low blood sugar, gallbladder issues, pancreatitis, kidney problems, worsening gastrointestinal symptoms, or other complications.

Seek emergency medical care immediately or call 911 if you experience:

Swelling of the face, lips, tongue, or throat

Any symptom that feels urgent, severe, or life-threatening
VitalCare Medical Spa is not an emergency medical facility.
Patients must follow all instructions regarding medication storage, dosing, injection technique, missed doses, sharps disposal, follow-up visits, and safety monitoring.
Do not share medications with anyone. Do not change your dose without provider approval. Do not use expired medication, improperly stored medication, or medication that appears contaminated or unusual.
Some weight loss services require patient-specific medications, hormones, vitamins, supplies, prescription review, provider time, or pharmacy coordination. Once medications or patient-specific products are ordered, compounded, shipped, prepared, allocated, or dispensed, they may not be refundable, returnable, reusable, or transferable.
Late cancellations, late rescheduling, no-shows, or failure to complete required forms may result in loss of deposit and/or a fee of up to $300, depending on the treatment, products ordered, and clinical time reserved.
By participating in weight loss services at VitalCare Medical Spa, I understand that results are not guaranteed, risks and side effects may occur, medications may not be appropriate for everyone, compounded medications are not FDA-approved, and I am responsible for following all medical instructions and reporting any side effects or health changes.

These services are intended to support wellness and hydration goals when medically appropriate, but they are not a substitute for emergency care, primary care, diagnosis, or treatment of serious medical conditions.
IV therapy and vitamin injections are not appropriate for everyone. Patients must complete required intake forms, disclose accurate medical history, and receive any required provider review before receiving treatment.
VitalCare Medical Spa reserves the right to deny, delay, modify, or discontinue IV therapy if treatment may be unsafe, inappropriate, outside the clinic’s available scope, or not medically recommended.
IV therapy results are not guaranteed. Individual responses vary depending on hydration status, nutrition, medical history, medications, metabolism, kidney function, liver function, cardiovascular health, illness, lifestyle, and other personal factors.
VitalCare Medical Spa does not guarantee relief from fatigue, headaches, hangovers, illness, dehydration, inflammation, immune concerns, beauty concerns, athletic recovery, or any other symptom or condition.
IV therapy at VitalCare Medical Spa is not emergency care and should not be used as a replacement for urgent medical treatment.
Call 911 or go to the nearest emergency room if you have chest pain, trouble breathing, severe weakness, fainting, severe dehydration, confusion, stroke-like symptoms, severe allergic reaction, uncontrolled vomiting, severe abdominal pain, or any life-threatening symptom.
Before IV therapy, patients must disclose:
Medical conditions
Heart disease
Kidney disease
Liver disease
High or low blood pressure
Pregnancy or breastfeeding
Diabetes
History of fainting
Blood clotting disorders
Allergies
Current medications and supplements
Prior reactions to IV fluids, vitamins, medications, injections, or preservatives
Recent illness, hospitalization, surgery, or infection
Failure to disclose accurate information may increase risks and may result in denial of treatment, cancellation, loss of deposit, or termination of services.
Possible risks of IV therapy or injections include:
Pain or discomfort
Bruising
Bleeding
Swelling
Redness
Vein irritation
Infiltration
Phlebitis
Infection
Dizziness
Fainting
Nausea
Headache
Fluid overload
Electrolyte imbalance
Allergic reaction
Medication or vitamin reaction
Worsening of an underlying medical condition
Need for emergency care

Starting an IV requires vein access. VitalCare Medical Spa cannot guarantee successful IV placement in every patient.
If IV access cannot be safely obtained, the provider or practitioner may stop the attempt, recommend another service, reschedule, or refer the patient to another medical provider if needed.
Once an IV treatment has been started, mixed, prepared, opened, or administered, it is generally non-refundable.
Some IV therapy services require advance preparation of fluids, vitamins, minerals, sterile supplies, additives, and provider time. Once supplies are opened, prepared, mixed, or allocated to a patient, they may not be reusable for safety and sterility reasons.
Late cancellations, late rescheduling, no-shows, or failure to complete required forms may result in loss of deposit and/or a fee of up to $300, depending on the treatment, supplies prepared, and clinical time reserved.
Patients should follow all aftercare instructions provided by VitalCare Medical Spa. Patients should contact the clinic with non-emergency concerns and seek emergency care for serious symptoms.
Possible normal post-treatment effects may include mild soreness, bruising, tenderness, or temporary redness at the IV site. Symptoms that worsen, spread, or feel unusual should be reported.
By receiving IV therapy or wellness injections at VitalCare Medical Spa, I understand that the treatment has risks, results are not guaranteed, IV therapy is not emergency care, and I am responsible for providing accurate health information and following all instructions.
