Membership Agreement



Memberships

1. Essential Wellness

• Monthly Fee: $ 39.00

• Benefits: ONE(1) Elite (IM) Intravenous Muscular Injection every month 

• 15% discount on additional regular price IM and IV Treatments

• Excludes : IV Therapy treatments ; this membership is only for IM (Intramuscular Injections)

2. Advanced Wellness

• Monthly Fee: $139.00

• Benefits:  (1) IV Therapy Treatment monthly  

• ONE (1) Vitamin Booster monthly 

• 15% discount on regular price IM and IV Treatments

• ONE (1) Complimentary (30m) consultation with our RN quarterly

• Excludes: NAD Therapies.

3. Ultimate Wellness

• Monthly Fee: $299

• Benefits: TWO (2) IV Therapy Treatments monthly.

•  THREE (3) Vitamin Booster monthly.

• 15% discount on regular price IM and IV Treatments

• ONE (1) Complimentary (30m) consultation with our RN quarterly

• Excludes : NAD+, Weight Loss Injections

Terms and Conditions

Membership Agreement

1. Membership Duration: Memberships are billed on a monthly basis with a commitment of Six (6) months. After the initial 6-month period, this membership will NOT renew automatically. 

2. Billing: Membership fees are charged monthly on the 1st of each month to the credit card on file.

3. Service Rollovers: Unused services will not roll over to the following month. However, members can give away their service treatment to friends and family without any additional charge. Service IV or IM treatment will be the same as scheduled and approved by the nurse at the initial consultation. Rollover changes must be approved by the Nurse on staff. An appointment must be made, and the patient must speak directly to the manager on staff and clear any changes 48 hours prior to the appointment. If patients do not call and no-show to their scheduled appointment, they will forfeit their service for the month.

4. Discounts: Membership discounts apply only to treatments and services listed in the membership benefits. Discounts cannot be combined with other promotions or offers.

Cancellation Policy

1. Notice of Cancellation: Members wishing to cancel their membership within the initial 6-month commitment must provide notice at least 60 days prior to the desired cancellation date. Notice must be sent to the manager on duty by email, phone call, or in person. TEXT MESSAGES will not be accepted as a form of cancellation. After the first six-month commitment has been fulfilled, the contract will not renew automatically. 

2. Cancellation Process: Notice can be sent via email to [INFO@VitalCareMedspa.com] or delivered in person at [8317 Gunn Hwy, Tampa FL, 33626].

3. Final Billing: Upon receiving a cancellation notice, the member will be billed for the remaining two months of membership fees. Membership benefits will continue until the end of this period.

4. Non-Refundable Fees: Membership fees are non-refundable. Members are encouraged to utilize all benefits during the notice period of (30days) or gift them to family or friends.  No refunds for the month of the appointments will be issued at any time, these treatments were specifically tailored and ordered just for your treatment wich come with a very short shelf life.

Changes to Membership

1. Upgrade/Downgrade: Members may upgrade their membership level at any time with a 30-day notice. The new membership fee will take effect from the next billing cycle.

2. Notification of Changes: Any changes made by the Clinic to membership benefits or terms will be communicated to patients at least 30 days in advance by email.
